I've added this to my theme as a workaround
.ui-corner-all > .ui-header {
-webkit-border-top-left-radius: inherit;
border-top-left-radius: inherit;
-webkit-border-top-right-radius: inherit;
border-top-right-radius: inherit;
}
In this case, I can probably move the <script> outside of the data-role="page/dialog"
thought this might be helpful if anyone is having issues with rounded corners
