Dynamically create form elements

Dynamically create form elements

I downloaded the code below to allow the user to add another form element if required. It works BUT it places the text from the previous text box into the new one.
 
I want to create an email page where the user can create as many recipents as possible, but this isnt acceptable. How do i modify the code to remove the clone and just create a new form element?
