Any Plugins to create conditional Questionnaire using jQuery?

I would like to create a conditional Questionnaire using jQuery which will finally lead to an end result based on the choices made by the person answering the questions.


Can anyone tell me about a jQuery plug-in?
Or can this be done using simple jQuery statements and conditions?
