app closes

Hello
I've made an app with phonegap which opens a webpage.
I make a php page with jquery mobile and upload it to the address which I have written in phonegap.

When open my app on the phone sees it fine, but when I want to write something in a form field shut it down.
Someone who knows what I'm doing wrong?
