Hi, all!  I have this little problem, which I can't find an answer to.  I have a form with a datepicker and a couple of other select fields.  However, when I submit it, my datepicker info is not being passed along, only the other selects.  I;m attaching the code in the pastebin:

I tried to remove the  noscript part, use div instead of input, but to no avail.  Do you guys see anything out of the ordinary?
