Form within a jQuery modal

Hi guys,

I have been using modals for simple effects however now wish to have a form within a modal - Ideally i'd like the user to complete the form & once they click the submit button the modal closes and redirect them back to a 'results' page.

Can anyone suggest how this can be done or point me in the right direction?
