Format plugins stopped working

I use formatcurrency and a phone number formatting for this form: http://youradminpartners.com/sales-contact-entry/ Everything worked fine.

I added a checkbox field and suddenly the formatcurrency and phone formatting stopped working. I took out the checkbox field but no good. I had saved a copy of the file before I added the field and went back to it. No good.

What the hell happened?
