[jQuery] datePicker is gone when 2nd time loading a form

[jQuery] datePicker is gone when 2nd time loading a form

and now for the solution :/
$_SESSION['count'] ++;
and every form input gets a raising id, so that its unique everytime -> datepicker works :<
