looking for a jQuery Form -

Glad to see a jQuery forum - I love this jquery stuff! Anyway, I am looking for a form that is really cool. I need this form for a lot of user input, more than just name email comment - I have found a few and they are kinda cool but I am not an expert at this either.. I know a lot but run into a little trouble adding fields in the PHP part of the code. I guess my question is really.. is there a user friendly jquery example downlandable code that is easy to understand? I can and will spend some time on these forms but need a little help... any form advice is appreciated - my client will thank you too!
