My css autocomplete doesn't work in modal bootsrap, please review !

hi guys, i have been stuck in css autocomplete please don't know how to debug it,

Please after you loggin then trying in first div just type in the 2 input type,my autocomplete working well there then press icon plus modal will be dissapear
