I am totally new to jQ and JavaScript.  My friend wants me to put a  Horizontal Sliding Contact Form on her website. She wants the form to look like this:

I went into the source and got the .js & .css files that I think I need to create the form.  I did a copy & paste of the code that I think is relevant to the form but it doesn't show up at all when I preview the page in a browser (Safari, Flock, Opera).  As I am a newbie, I have no idea of what is going on, so I'm hoping to get some help here.  In the meanwhile I am doing tutorials, but I would love to get this done asap.  Thank you for your help!
