Could someone help me for this :  Code on FiddleJS

I try to do : 

when switch1 or switch2 is on ' Yes' (Block1 or Block2 is showed) but switch3 should be on ' No' and locked on 'No' and block3 hidden.

when switch3 is on ' Yes' (Block1 and Block2 are hidden) and switch1 and switch2 turned on ' No' and locked (disabled).
