Weirdness displaying radio button after form submissions

After submitting a form, we're displaying all the form field data, but for some reason, everything after the radio button will not display on the page.

  1.   var display =
  2.     "<h4>Client room</h4>" +
  3.     $("#firstName").val()         + " " +
  4.     $("#lastName").val()          + " " + "<br>" +
  5.     $("#emailAddress").val()      + " " + "<br>" +
  6.     "Sample 1: "              + $("#q1").val() + "<br>" +
  7.     "Sample 2 "            + $("#q2").val() + "<br>" +
  8.     "Existing: " + document.getElementById("q3").value; + "<br>" +
  9.     $("#survey1").val()      + " " + "<br>" +
  10.     $("#survey2").val()      + " " + "<br>" +
  11.     $("#survey3").val()      + " " + "<br>" +
  12.     $("#survey4").val();
  13.   $("#result").html(display);
  14.
  15. <div class="form-group">
  16.     <div class="radio">
  17.     <label><input name="q3" id="q3" value="Yes" type="radio">Yes</label>
  18.     </div>
  19.     <div class="radio">
  20.     <label><input name="q3" id="q3" value="No" type="radio">No</label>
  21.     </div>
  22. </div>
  23.
  24. Results looks like this:
  25. John Doe
  26. johndoe@testtesttest.com
  27. Sample 1: Textile
  28. Sample 2: Wood
  29. Existing: Yes
  30. (All the survey questions should be listed here.)
Earlier I tried this, but it returned "Undefined":
  1. $("#q3").val()
