Incorrect radio buttons selected when inserted dynamically
in jQuery Mobile

When I insert radio buttons dynamically, the new radio buttons only activate the old radio buttons. I have been banging my head against the wall on this for a day now, and am no closer to a resolution. I have posted sample code at http://johnwoltman.com/a/1.html and maybe someone can tell me what I'm doing wrong.

Sorry, it was kind of hard to follow everything going on in your code... So I hope this can help. I created a fiddle with a working example of dynamic radio buttons that hopefully you can follow. Perhaps then you may be able to figure out where you went wrong? I would make sure of the following.
- Your names for each group are different. It appears to me as though the names are possibly the same, resulting in the strange behavior.
- All IDs are unique
- You are creating a new fieldset for the groups.
If that is the case, it should be okay.

Your fiddle example is very clear, but I think I'm doing everything correctly. When I list out all the names and IDs, I see that they have all been updated by the code in jquery.formset.js.
Do you think there's a refresh or trigger step I'm missing? I referred to the JQM docs, but the .appendTo().trigger("create") trick doesn't seem to be working for me.

You're using a plugin intended for jQuery: jquery.formset.
I would not have any expectation that this would work with jQuery Mobile.
Your page structure is really odd, and I would expect this to cause trouble, as well. Your form tag is outside of the page div, so at best this is going to work with only a single-page site. You don't have any data-role attributes: you apparently marked-up the jQuery Mobile classes (ui-page, etc.) by hand.
Maybe you are confusing jQuery UI and jQuery Mobile? jQuery-UI CSS classes look similar to those of jQuery Mobile.
The jquery.formset script has a 2009 date indicated in the source, which I believe predates even the existence of jQuery Mobile.
You might get this to work, with sufficient knowledge of jQuery Mobile and some modification. I would instead seek a solution designed for jQuery Mobile.
I would not have any expectation that this would work with jQuery Mobile.

Watusiware, thank you for noticing my bad example 
I uploaded a better (the correct!) version of my sample code. The original sample code was post-dynamic content, this new version has the raw markup and it passes the w3c validator, as well as having the form tag in the correct place.
The date on the jquery.formset file is inaccurate - I am using a trunk build which still receives updates. It provides a hook to call a function after it inserts input elements, but I can't figure out what (if anything) to do with the hook. The problem still manifests itself on my updated example.
