The icon files and load files are expected to be found in an /images directory directly under whatever directory contains the .css. For your current directories, that would be:

You can get the images from the .zip file on the download page, or you can get them from a link at the bottom of the download builder. (Download builder is not the same as Themeroller.) However, the download builder is for advanced use, and I do not recommend it unless you know what you are doing and really need to reduce size. You can still use the convenient link at the bottom, though, to get the image files. 

You will need to consult the Cordova documentation in case there are any issues with permissions for the /images directory, etc.
